• Ship to address (for the test detector(s) return delivery).
• Method of return shipment (the costs below do not include shipping costs).
  If nothing is stated, NIST will return by common carrier, collect, and uninsured.
• User's name, phone number, and email address.
• Name and address of person to receive the calibration/test report/s.
• Service ID number(s).
